What the undergraduate application form asks for, the documents to have ready, and what happens once it is submitted.

What this form is

The undergraduate application form is the single document that opens your file at Bayan University. One form covers every undergraduate programme in the three colleges — you name the programme on the form rather than filling in a different form for each one. The form decides nothing by itself; it starts a file that the department then assesses against its own entry requirements.

What the form asks for

The form is short, and every part of it is used. Answer it in the same names and spellings that appear on your documents.

Personal details

Full name exactly as it appears on your identity document, date and place of birth, nationality, and a permanent address.

Contact details

A mobile number you answer and an email address you read. Everything the admissions office sends after submission goes to those two, so give the ones you use yourself rather than a relative's.

Programme choice

The college and department you are applying to, and a second choice if you want one considered. The programme pages set out what each department teaches and the award it leads to.

Education history

The school you finished, the year of the certificate, and the result. If you studied outside the Kurdistan Region or Iraq, say so here — that file needs an equivalency decision before it can be assessed.

Declaration

A signed statement that the information is true and the documents genuine. A false declaration voids an offer at whatever stage it comes to light, including after enrolment.

Documents to have ready

  • Secondary school certificate and its transcript
  • National identity document or passport
  • Recent photographs at the size the form specifies
  • Equivalency decision, where the certificate was awarded outside the region
  • Anything the department page names as an additional requirement

Scan or photograph each document whole and legible. Cropped corners and dark photographs are the commonest reason a file sits waiting.

Submitting it

Start the application online, or complete a paper copy at the admissions office and hand it in with the documents. Both routes reach the same desk and are assessed the same way.

After you submit

  1. The office checks that the file is complete and asks you for anything missing.
  2. The department assesses it against the entry requirements for that programme.
  3. You are invited to an interview or a placement test where the programme requires one.
  4. You are told the outcome, and an offer is followed by registration.

Keep the reference you are given at submission. Quote it whenever you contact the office about your file — it finds your application faster than a name does.
